At APS Healthcare, our mission, quite simply, is to make a significant and positive impact on the healthcare system by changing behaviors and improving outcomes. We are managing a new Care Coordination Management program for the California Department of Health Care Services (DHCS), servicing seniors and persons with disabilities and those with severe mental illness. The goal of the APS Care Coordination Management program is to decrease the fragmentation of care, improve health outcomes and end-of-life decision making along with reducing healthcare costs.
We want you to help us make your community healthier and better! We are looking for experienced RN Health Coaches to join our team. The Community Based RN Health Coach works with high risk Medicaid beneficiaries to improve health and prevent hospitalizations, working in various hospitals, health centers, APS field office and, in certain instances of high acuity, a member’s home. All positions require a valid, unrestricted CA RN license and at least 3 years of nursing experience, which must include some psychiatric experience. Previous care management (disease management, case management, discharge planning) is strongly preferred. Positions are typically Monday – Friday with no weekends or holidays. We have a variety of field based positions throughout various CA counties below. All require travel within the assigned communities and to the Sacramento office for training.
